SMS Consent & Opt-In Policy

This page documents the patient consent workflow used by dental practices utilizing Molaris. It is intended for patients, dental practices, and telecommunications carrier compliance review.

We do not send promotional messages. Transactional form links only.

Consent is Optional

Patients who decline SMS receive their forms by email instead.
Receiving dental treatment is NOT dependent on SMS consent.

Who sends what

Molaris is a HIPAA-focused communications platform used by dental practices. Molaris never independently contacts patients or markets directly to them. All SMS communications are initiated only after the patient’s dental practice obtains the patient’s consent. Molaris never independently collects patient phone numbers and only delivers transactional messages after the dental office records that consent.

The patient gives their number and consent to their own dental office — never to Molaris directly.

Consent workflow

  1. Patient requests forms by text while scheduling or checking in.
  2. Staff ask permission using the spoken disclosure below.
  3. Patient agrees — verbally, or by initialing the paper intake form.
  4. Staff record the consent in the patient’s record in Molaris.
  5. Molaris sends the SMS — only then, and only about that patient’s forms.

How it happens at the front desk

Patient checks in
Staff: “Would you like us to text your intake forms?”Full disclosure read aloud — rates, frequency, STOP/HELP
Patient: “Yes”
Staff record consent in the patient’s chart
Molaris sends the form link by SMS

Visual proof of the consent workflow

The three artifacts below show exactly how consent is collected and used. (Representative mockups — each practice’s branding varies.)

Screenshot 1 · The paper intake form

Offices collecting consent in writing use this consent block on their paper intake form:

PATIENT INFORMATION
Phone Number:
I agree to receive SMS messages from my dental practice (sent via Molaris) regarding my appointments, forms, and dental care.
Message frequency varies. Message and data rates may apply.
Reply STOP to opt out. Reply HELP for help.
Consent is not required to receive treatment.

Screenshot 3 · What the patient receives

Up to three transactional message types, each tied to a form the patient asked to receive by text — typically 1 to 3 messages per visit:

Text Message · Today 9:41 AM
[Dental Practice Name]: Please review and sign your medical history update before your visit: https://www.getmolaris.com/f/x2… Reply STOP to opt out, HELP for help.
[Dental Practice Name]: Your intake form still needs your signature before your appointment. Reply STOP to opt out.
[Dental Practice Name]: Thank you, we received your signed form. No further action is needed.
If the patient replies STOP:
STOP
[Dental Practice Name]: You are unsubscribed and will receive no further messages. Your dental care is not affected.

We never send marketing or promotional messages, and we never message purchased, rented, or shared lists.
